San Juan Basin Public Health and Community Connections Inc. have announced an upcoming change to single point entry case management services in Archuleta, La Plata and San Juan counties.
Effective July 1, CCI will serve as the single point entry case management agency for these three counties and SJBPH will end single point entry operations.
Single point entry case management agencies coordinate services needed by people who are elderly or have a disability to safely remain in their homes and communities for as long as possible. People who are currently enrolled in services will not lose or have changes to their benefits or services because of this change.
SJBPH and CCI are working collaboratively to ensure a smooth transition for members, with recognition that older adults and individuals with disabilities are at heightened risk during this pandemic. Additionally, CCI is coordinating with SJBPH to employ all eligible case managers in an effort to maintain the working relationship between members and their current case managers through this transition.
All current members and service providers have been notified by mail of this change and do not need to take further action.
For more information, call SJBPH at 247-5702 or 259-2464.
